Application Guidelines
Perfect safety records aren't required — recognition is based on whether your company's incidence rate is better than the National Bureau of Labor Statistics (BLS) injury and illness rate for your NAICS code.
For companies with multiple facilities, each location can earn its own award. Please submit a separate OSHA 300A form for each facility.
Award Categories
- Honorable Mention = Incidence rate up to 10% better than BLS
- Award of Merit = Incidence rate more than 10% better than BLS
- Award of Honor = No reportable injuries
- Most Improved = Greatest percentage of improvement over the prior year
Award Winners
ALL company-level members who qualify at any award level will receive a plaque and be promoted on FMA's media outlets. They will also be invited to accept their award at a special reception and dinner honoring Safety Awards winners during FMA's Safety Conference.
